Selbstauskunft und Frage zur kurberatung

Hallo,

ich habe einen ´Termin bei derKurberatung (Müttergensungswerk) und habe den ANtrag jetzt hier liegen, auch schon teils ausgefüllt, aber irgendwie tue ich mich mit den Formuliereungen usw... ziemlich schwer, auch was meine Tochter betrifft, da ich sie als Therapiekind mitnehmen möchte. Helfen die einem bei dem AUsfüllen des ANtrages, was ich jetzt irgendwie nicht hinbekomme?

Und der Selbstauskunftsbogen, wieviel habt ihr da geschrieben?

Hallo Sandra,
in der Selbstauskunft kann man gar nicht zu viel schreiben. Beschreibe einfach Deinen Tagesablauf, was Dich belastet und einschraenkt und alles, was die KK braucht, um zu verstehen, dass etwas schieflaeuft und du Hilfe brauchst.
Fuer die Kinder hat die Atteste mein Kinderarzt ausgefuellt, da musste ich gar nichts tun...
Aber die Kurberatung hilft Dir in jedem Fall und beantwortet Deine Fragen!

Du musst da auch nicht in hochgestochener Fachsprache daherkommen. Wenn du es mit deinen eigenen Worten beschreibst, reicht das vollkommen aus. Wenn du auch damit schwer tust, dann kann die Beratungsstelle dir dabei helfen. (kommt auf die Mitarbeiter dort an) Aber fragen würde ich schon. Nicht jeder kommt mit solchen Formularen klar.
